FormIndex is a small independent product run by its own team, working from Rooms 1518–19, 28/F, Enterprise 168, Sheung Yuet Road, Kowloon Bay, Kowloon, Hong Kong. That team is the data controller for the personal data described in this policy. Questions about it are welcome at [email protected], which is read by the people who built the site rather than a support desk.
We do not collect special category data, and we do not ask for your name, phone number, or postal address.
We use a small number of strictly necessary cookies to keep you signed in and to remember your plan. These cannot be switched off without breaking the service.
This site also uses the Meta (Facebook) Pixel. If you arrived here from an advertisement, the Pixel records that visit and any sign-up, and reports it back to Meta so we can measure whether our advertising works. This means Meta may receive your IP address and details of your activity on this site, and may match it to a Meta account if you have one. Meta acts as an independent controller for that data — see Meta's own privacy policy for how they handle it.
You can opt out through your browser settings, your Meta ad preferences, or by declining non-essential cookies where we offer that choice.
We share data only with providers who process it on our behalf, under contract, and only as far as needed:
We do not sell your personal data, and we do not share it with data brokers, advertisers, or any wagering operator.
Some providers operate outside your country. Where data leaves the UK or EEA, we rely on the UK/EU Standard Contractual Clauses or an adequacy decision.
Account and usage data is retained while your account is active and for 12 months afterwards. Billing records are kept for as long as local accounting rules require, which is currently seven years. Server logs are deleted after 90 days.
Depending on where you live, you may have the right to access, correct, delete, restrict, or object to our use of your data, to receive a portable copy, and to withdraw consent. Email [email protected] and we will respond within 30 days. You can also delete your account and all associated data from your account settings at any time.
If you are in the UK or EEA and you are unhappy with our response, you may complain to your national data protection authority.
FormIndex is not intended for anyone under 16. We do not knowingly collect data from children. If you believe a child has given us data, contact us and we will delete it.
If we make a material change to this policy we will email registered users at least 14 days beforehand. The effective date at the top always reflects the current version.

By creating an account or using FormIndex you agree to these terms, which form a contract between you and the team that operates FormIndex, at the address given in our Privacy Policy. If you do not agree, do not use the service.
FormIndex is a data and visualisation tool. It presents historical and current sports statistics: team form, player metrics, and head-to-head records.
FormIndex does not provide predictions, forecasts, tips, recommendations, or advice of any kind. Nothing in the service is an inducement or suggestion to take any action, financial or otherwise. Every interpretation of the data, and every decision you make from it, is entirely your own. We are not affiliated with, sponsored by, or connected to any wagering operator, and we receive no commission from any such party.
You must be at least 16 to use FormIndex. You are responsible for keeping your login secure and for everything done through your account. Tell us promptly at [email protected] if you think it has been compromised.
You may use FormIndex for your own personal or internal business analysis. You may not:
Our data comes from third-party providers. We take care in processing it, but we do not warrant that it is complete, accurate, or free from error or delay. Leagues sometimes amend official results after the fact. FormIndex is not an official record of any competition — for that, consult the competition organiser.
The FormIndex software, interface, brand, and our derived metrics (including the Form Index) belong to us. Team names, competition names, and club marks belong to their respective owners and are used descriptively; we claim no affiliation with them.
The service is provided "as is" and "as available". To the fullest extent the law allows, we exclude all implied warranties, and we are not liable for indirect or consequential loss, lost profits, lost opportunity, or any decision you take based on the data.
Where liability cannot be excluded, our total liability to you in any 12-month period is limited to the greater of the fees you paid us in that period or USD 100. Nothing here limits liability for death, personal injury, or fraud.
We may suspend or close an account that breaches these terms, with notice where practicable. You may close your account at any time from settings.
We may update these terms. For material changes affecting paying subscribers we will give at least 30 days' notice by email; continuing to use the service afterwards means you accept the new terms.
These terms are governed by the laws of Hong Kong, and the courts of Hong Kong have exclusive jurisdiction. This does not remove any protection you have under the mandatory consumer law of your own country.

Match results, fixtures, and player statistics are supplied under licence by a commercial sports-data provider, who source them from official competition data feeds. Historical archives are reconciled against published league records before being added.
In-play and post-match data is ingested continuously and typically appears within minutes of the final whistle. Derived metrics — including the Form Index — are recalculated after every completed fixture.
The Form Index is our own calculation, not a figure published by any league. It is a rolling score over a team's last ten completed matches, weighted by the strength of each opponent at the time the match was played, so that a draw away to a strong side counts for more than a win at home to a weak one. It is descriptive of past results only and is not a forecast.
Competitions occasionally amend official results after publication — for disciplinary decisions, scoring corrections, or awarded matches. When our provider issues a correction we apply it and recalculate any affected metrics. If you spot something that looks wrong, tell us at [email protected] and we will check it against the official record.
